Create a survey flow with an add-on

This article provides guidance on how to convert a product-attached survey into a streamlined breadcrumb-style survey flow that appears before checkout, includes an optional add-on offer, and passes all collected data through to the checkout.

Create a product page with the following URL: /green-box-standard-survey for the product @Green Box Standard. The page should include a breadcrumb-style menu for the survey/quiz attached to this product (Survey ID: 17403). The flow should include the following steps: Survey Steps: Display all steps from the attached survey. Plan Selection: After completing the survey, prompt the customer to select one of the plans available for the @Green Box Standard product. Optional Add-On: The final step should present the optional add-on product @Fresh Boost Pack. For this step, display two buttons: Add to my box – if the customer clicks this, add the optional add-on product to the cart. No, thanks – if the customer clicks this, skip the add-on and lead the customer directly to the cart. This step is optional. Ensure that all previously selected preferences from earlier steps are collected and passed to the cart/checkout, so that the cart reflects all customer choices correctly. Checkout: Once all steps are completed, redirect the customer directly to the cart/checkout page with all selected options and preferences saved and passed to checkout.
After completing all steps, when I click either "Add to my box" or "No, thanks" on the final step, the cart is not opening and I'm redirected to the /cart page. Here's how it should work: If I click "Add to my box", the cart should open showing both the previously selected plan and the optional add-on. If I click "No, thanks", the cart should open showing only the previously selected plan, without the add-on. All previously selected preferences must be properly passed to the cart/checkout.
Remove add-on duration
Include header and footer for this page, and also better style survey cards. We can include some images instead of check and radio buttons and make them as pickable cards
